Request Medical Records

Stay Informed About Your Health

Every patient can request a copy of his or her medical records. At Valley Baptist Health System, we make it easy to request your medical records to use for further care or to stay informed about your health.

A person can request their medical records by:

  • Completing an authorization form
  • Requestor must present a valid Texas form of ID
  • Request should be mailed in to Health Information Services, 2101 Pease St., Hgn. TX 78550 or can be brought in person.

Your physician’s office may request a copy of your medical record by faxing the form to (956) 389-1714.

Online Patient Portal

You can register for the Valley Baptist online patient portal to access lab results, medical records and pay bills online. Learn more about our patient portal.
