Membership Number
Each member of the Facility is issued a membership card.
Members check in with their card at the Member Services Desk to
enter Valley Baptist Health & Fitness Center. Lost or damaged cards must
be replaced – members will be charged $5.00 to replace card.
Dues Policy
Members will make payments in full on the 1st of every month.
Options for paying are to pay for the year in advance with a %5
discount or have dues automatically charged to the member credit
card or drafted from their checking/savings account at the
beginning of every month. Payroll
deduction is available through Valley Baptist Health System for VBHS employees if you would like to take advantage of this
option. Accounts may be frozen if members bring in a
written note from their attending physician. No refunds on gym
memberships.
Guest Policy
A guest must be accompanied by a member (18 or older); the
guest must register and pay a guest fee at the Member Services
Desk prior to using the facility. A guest is also required to
complete a Physical Activity Readiness Questionnaire (PAR-Q) and
an Assumption of Risk if results deem it necessary. Arrangements
for unaccompanied guest must be made in advance with a member of
the staff. Guest fees are determined by the length of the stay.
Member Age Restriction
The minimum age for a monthly gym and/or programs membership
is 12 years of age. Members under the age of 16 may use the
facility only after a complimentary 1 on 1 training with a staff
member. Kids Fitness Classes are held throughout the year at
schedule times for ages 6-11.
Emergency Procedures
Should an accident occur at Valley Baptist Health & Fitness
Center,
immediately report it to a staff member on duty. Emergency
procedures will be expected immediate. Should you or a family
member become injured at Valley Baptist Health & Fitness Center, staff is not permitted to
provide off site transportation.
Lost and Found
Valley Baptist Health & Fitness Center is not responsible for lost or
stolen items. Lost and stolen items inquiries should be made
thorough the Member Services Desk. Items turned in and not
claimed after thirty (30) days will be donated to charity.
One on One Training
Each member of Valley Baptist Health & Fitness Center is eligible to receive a
Complimentary 1 on 1 training session in order to evaluate
Fitness and Health Status in regards to program design. This
value added service allows Valley Baptist Health & Fitness
Center staff to assist the
member in specific, important way. Staff will train member in
use of strength training equipment and cardiovascular equipment.
Staff will also state important principles of exercise,
including specificity, progression and overload.
Day Lockers
Membership is asked to respect Valley Baptist Health &
Fitness Center daily locker service.
Please do not bring valuables into the facility or leave in
lockers. Valley Baptist Health & Fitness Center is not responsible for items lost, stolen or
damaged. Lockers will be cleaned out at the end of each day and
contents thrown away or donated to charity.
Towel Service
Membership is asked to respect Valley Baptist Health &
Fitness Center willingness to
provide members with towels by returning all towels in the
correct dirty linen bin.
Dress Code
Shorts, sweat suits and athletic shorts must be worn at all
times in the gym area. Members wearing inappropriate clothing
will be asked to leave.
Other Rules
- Valley Baptist Health & Fitness Center has the right to refuse
service to anyone for violating House Rules or any other
just cause. If services have been paid, the unused balance
will be refunded and membership canceled.
- Employees of VBHS or related entities agree not to
participate in this program during the course and stop of
their working hours.
- Be courteous and wipe equipment down after use if
needed. Towels are complimentary while working out.
- Members are liable to Valley Baptist Health & Fitness
Center for
the value of all facility property which shall be damaged or
removed by them or their guests.
Complaints and Suggestions
All complaints and suggestions should be submitted in writing
to Valley Baptist Health & Fitness Center management. Complaints and
suggestions shall be placed in the suggestion box located at the
Member Services Desk. We encourage members to include their name
and phone number with all comments so that we may contact you
for specific feedback. Be courteous with suggestions and
feedback please.
Cardiovascular Equipment
- Instruction in the proper use of equipment is the
responsibility of each member.
- Proper care and use of all equipment is the
responsibility of each member.
- Valley Baptist Health & Fitness Center disclaims any liability
for unauthorized or inappropriate use of the equipment and
facilities, which results in physical injury or other bodily
harm to members or their guests.
- Please keep cardiovascular equipment wiped down after
each use or let a staff member know it needs to be
sanitized
