We regret that you have experienced a change in your health which has required you to seek “acute” medical attention. We are glad you have selected Valley Baptist to provide you with your health care needs.

Your stay here is to address your “acute” needs, and we will develop a partnership with you to provide you with the care as ordered by your physician(s). Your full participation in the treatments your physician ordered is anticipated to help you recover from your “acute” health status. Once your physician determines that the delivery of the prescribed treatment has improved your medical condition, and that the “acute” phase of your health care needs have passed, your physician will write orders for your discharge from Valley Baptist Medical Center.

The discharge plan orders may include orders to return to your home and follow-up in your physician’s office, admission to a nursing home, transfer to an inpatient rehabilitation center or transfer to another facility that specializes in delivering care for your special health care needs. Your physician may order other services to assist you at home; these may include home health services, providers, oxygen, wound care etc. As necessary, a Case Manager/Social Worker will meet with you and your family/friends to ensure appropriate plans are made for your discharge.

There are many providers in the community available to help with your continuing care needs beyond the hospital. And the choice is yours. We would like you to know that Valley Baptist Health System is proud to be able to offer continuing care beyond the hospital setting – in our Rehab Unit, Skilled Nursing Facility (Golden Palms), Home Health / Hospice, Dialysis Center (both Harlingen & Raymondville), Wound Care Clinic, Jessie Crump Diabetes Center, Outpatient Rehab Clinic and through our Durable Medical Equipment supplier (Advanced Medical Supply).

Your participation in the discharge plan is crucial. We anticipate and expect that you and/or your family members and friends will be actively involved in helping to make and complete the discharge plan as ordered by your physician. We need your full cooperation and preparedness to follow your physician’s discharge plan in a timely manner. Therefore, we request that you remain in constant communication with us, and your family and friends to make this happen. Your stay with us will be no less/no more than what your physician prescribes.
