Social Services and Care Management
The Care Management department at Valley Baptist is made up of Social Workers and Case Managers. These people assist the patient in understanding their diagnosis and plan of care. If a patient needs continued care after leaving the hospital, the Case Manager and Social Worker help the patient / family and physician in making sure necessary services are arranged. This includes working with health care providers and resources both within the hospital and in the community.
