The Care Management department at Valley Baptist is made up of Social
Workers and Case Managers. These people assist the patient in
understanding their diagnosis and plan of care. If a patient needs
continued care after leaving the hospital, the Case Manager and Social
Worker help the patient / family and physician in making sure necessary
services are arranged. This includes working with health care providers
and resources both within the hospital and in the community.
