Registration Information
Upon arrival at Valley Baptist Medical Center - Brownsville, you will be requested to provide the registration clerks with your insurance card and another form of identification, preferably a driver’s license. You will need to be prepared to make your co-payment at the time of admission if required by your insurance company. If you have been a patient here before, or have a Premiere Plus or another card to facilitate registration, we will simply verify information.
You will be required to sign the “Conditions of Admission” and “Advance Directives” forms at the time of registration. The “Conditions of Admission” form enables us to bill your insurance company directly. You are responsible for any deductible and costs not covered by your insurance, and we may ask that you pay a deposit to cover these costs.
Upon signing this agreement, it provides consent for general nursing care, diagnostic testing, and administration of medications as ordered by your physician.
As a patient at Valley Baptist-Brownsville you are entitled to considerate and respectful care in a safe and secure environment. You will be provided a copy of the Patient’s Bill of Rights upon admission.
The “Advance Directives” information explains your rights under state law to issue Directives to a physician and/or Durable Power of Attorney for Health Care. You are given the opportunity to execute such directives at any time.
There will be other forms requiring your signature depending upon your particular circumstances. If you are unable to sign these forms your spouse, next of kin or legal guardian may sign for you.
