This notice describes how medical information about you may be used and disclosed. It also allows you to gain access to this information, so please review privacy practices carefully.

Who Will Follow This Notice
Each of the employees and physicians on staff at Valley Baptist Health System, or any of the Valley Baptist entities, has agreed to abide by the terms of this notice when they perform their duties for us or in our facilities.

About this Notice
This notice will tell you about the ways we may use and disclose your medical information. We call this information protected health information, or “PHI” for short, and it includes information that can be used to identify information that we have created or received about your past, present or future health. We also describe your rights and certain obligations we have regarding the use and disclosure of PHI.

We are required by law to:
Make sure that PHI that identifies you is kept private; Give you this notice of our legal duties and privacy practices with respect to your PHI; and Follow the terms of the notice that is currently in effect.

Changes to this Notice
We reserve the right to change this Notice. We reserve the right to make the revised or changed Notice effective for PHI about you we already have, as well as any information we receive in the future.

Complaints
If you believe your privacy rights have been violated, you may file a complaint with Valley Baptist Health System or with the Secretary of the Department of Health and Human Services. To file a complaint with Valley Baptist Health System, please call or write to the Privacy Office. You will not be penalized for filing a complaint.
