Registration Information
Upon arrival at Valley Baptist Medical Center - Brownsville, you
will be requested to provide the registration clerks with your
insurance card and another form of identification, preferably a
driver’s license. You will need to be prepared to make your
co-payment at the time of admission if required by your
insurance company. If you have been a patient here before, or
have a Premiere Plus or another card to facilitate registration,
we will simply verify information.
You will be required to sign the “Conditions of Admission” and
“Advance Directives” forms at the time of registration. The
“Conditions of Admission” form enables us to bill your insurance
company directly. You are responsible for any deductible and
costs not covered by your insurance, and we may ask that you pay
a deposit to cover these costs.
Upon signing this agreement,
it provides consent for general nursing care, diagnostic
testing, and administration of medications as ordered by your
physician.
As a patient at Valley Baptist-Brownsville you are
entitled to considerate and respectful care in a safe and secure
environment. You will be provided a copy of the Patient’s Bill
of Rights upon admission.
The “Advance Directives” information
explains your rights under state law to issue Directives to a
physician and/or Durable Power of Attorney for Health Care. You
are given the opportunity to execute such directives at any
time.
There will be other forms requiring your signature
depending upon your particular circumstances. If you are unable
to sign these forms your spouse, next of kin or legal guardian
may sign for you.
Private Duty
Nursing / 24 Hour Sitter
If you or your family member require
or request additional nursing care (private duty nurse or
sitter), these arrangements must be made by you and/or your
family.
Your Room
We offer both semi-private
and private rooms. Private rooms are available on a first-come,
first serve basis. We will do our best to accommodate your needs
when you arrive. Patients in the Intensive Care Unit or
Emergency Care Center receive priority for transfer; however, we
do maintain a private room transfer waiting list, and the
admitting staff will notify you if a room becomes available.
All rooms are SMOKE FREE for the safety and comfort of our
patients. Smoking is permitted in designated areas outside the
hospital.
If you have any questions regarding your room,
please contact the admitting staff at (956) 698-5406 or (956) 698-5605, or
the Patient Advocate at (956) 698-5422.
Personal Needs
Toiletries, bathrobe, slippers and pajamas/gowns are the only
personal items that you need bring with you. You should also
bring any insurance materials, including insurance card and
claim forms, Medicare/Medicaid card, or Military ID (for Champus
claims). If you have eyeglasses, contact lenses or dentures,
please use your own protective containers for these items.
